In connection with the proposed transfer of title, application has been made to TITLE INSURANCE COMPANY to guarantee title, including guaranty against claims arising out of lien of debts, if any, of the above estate. The information requested below will enable the Company to ascertain if it is willing to issue this insurance.
1. State date and place of death of decedent
2. Has Will been probated or letters of administration been issued? State particulars, including the court of probate or administration
2a. Have release and refunding bonds been signed by all devisees and distributes?
3. State age of decedent at time of death
4. State the business or occupation, if any, of decedent at the time of death
5. State approximate gross value of estate. $

7. State the approximate amount of cash and liquid securities now owned by the estate. $
8. List the known unpaid debts, if any.
9. Have the funeral expenses and expenses of last illness been paid in full?

10. State whether Items 8 and 9 above will be paid at or prior to closing title.
11. Will New Jersey Transfer Inheritance Taxes, if any, been paid and release or waiver of tax procured?
12. Have Federal Estate Taxes, if any, been paid and release of waiver of tax procured?
13. Has the New Jersey Estate Tax, if any, been paid and release obtained?
14. Is any claim or action pending or threatened, to your knowledge, by any creditor? If so, state particulars.
15. If you wish to offer your personal indemnity to the TITLE INSURANCE COMPANY against possible debts in the estate, the following information is requested:
a. Your bank reference
b. Your net worth $
c. Your corporation, name and address of your employer, if any
The foregoing representations are within the personal knowledge of the undersigned and are submitted to the TITLE INSURANCE COMPANY to induce it to issue title insurance to a proposed purchaser or mortgagor free of any claim of debts in the above estate.
